Please see below frequently asked questions (FAQs) as well as fees and charges for eye examinations at Eye5 Optometrists.

What does it cost?

Examination fees 

Examinations at Eye5 Optometrists are not fully covered under Medicare and there will be a private fee for the testing that you cannot claim back from Medicare Australia or your private health fund.  We will ask you to sign a financial consent at your appointment.

What should I bring to my first appointment?

  • Your current glasses and/or contact lenses
  • If you want your contact lenses to be checked please come wearing them.
  • If you want a new prescription for glasses but also wear contact lenses please wear your glasses (at least for a few hours beforehand) but bring your contact lenses so we can check them too
  • Your Medicare card any private health insurance card

Is there anyone else at home that might need their eyes tested too?

Now that you have decided to have an eye test, can you think of anyone else at home who should have their eyes examined as well?

Word of mouth recommendations have allowed us to grow our practice and we love seeing and being able to assist friends and family of our existing patients. We would love it if you could help spread the word to your family and friends.

Where are you located?

Just off the Hutton street exit of the Mitchell freeway at 2/234 Main Street, Osborne Park.

Traffic on the Mitchell freeway heading north can make the trip take 10 minutes more than expected particularly in the afternoon.
